Gynecomastia is a condition caused by abnormal enlargement of the male breast gland that can seriously affect a person's self-confidence. With surgical treatment (liposuction and/or glandular tissue excision), a permanent and flat chest appearance is achieved.

What Is Gynecomastia?

Gynecomastia occurs due to an estrogen-testosterone imbalance that causes excessive development of the male breast gland. While it can appear temporarily during puberty, it may also become permanent in adulthood. Cases referred to as pseudogynecomastia are caused solely by fat accumulation.

In true gynecomastia, where diet and exercise are ineffective, surgical treatment is the only permanent solution. The procedure involves removing excess glandular tissue and/or aspirating fat tissue through liposuction.

Gynecomastia Classification

  • Grade 1: Mild enlargement; glandular tissue accumulation around the areola
  • Grade 2a: Moderate enlargement; excess that does not extend beyond the chest
  • Grade 2b: Moderate enlargement; skin excess has begun
  • Grade 3: Significant enlargement; notable skin excess and sagging

Who Is a Candidate?

  • Those with breast enlargement that has persisted for more than 6 months
  • Cases that have not resolved despite hormonal therapy or medication changes
  • Men who are embarrassed by their chest appearance and whose clothing and sports activities are affected
  • Individuals experiencing psychosocial impact
  • Men aged 18 or older whose general health is suitable for surgery
  • Non-smokers or those willing to quit

Gynecomastia Surgical Methods

Liposuction (For Fat Tissue)

In pseudogynecomastia or mixed cases, fat tissue is aspirated through small incisions via liposuction. It leaves minimal scarring and allows for rapid recovery.

Glandular Tissue Excision

Hard glandular tissue is removed through a semicircular incision along the areola edge. This is the primary treatment for true gynecomastia.

Combined Method (Most Common)

Both liposuction and glandular excision are performed together; a flat and contoured chest profile is achieved. Preferred for moderate to advanced cases.

Skin Resection for Grade 3

When significant skin excess is present, additional skin is removed through an extra incision; however, this leaves a longer scar.

Procedure Steps

1. Evaluation

The degree of gynecomastia, gland-to-fat ratio, and hormonal status are evaluated; an endocrinology consultation is arranged if necessary.

2. Anesthesia

General anesthesia or local anesthesia with sedation is administered. The procedure takes 1–2 hours.

3. Liposuction and/or Excision

Depending on the chosen method, fat is aspirated and/or glandular tissue is removed; the chest profile is flattened.

4. Compression and Closure

Incisions are closed; a compression vest is applied.

Before and After Surgery

Before

  • Blood thinners should be discontinued 1 week prior
  • Anabolic steroids and testosterone supplements must be stopped
  • Smoking must be stopped 2 weeks before

After

  • A compression vest should be worn during the day for 4–6 weeks
  • Avoid arm and chest exercises for the first 2 weeks
  • Alcohol and anabolic substances should be avoided
